Discharge is imminent, but what next? The transitional care team is on hand to help you and your confidants to be optimally prepared for the time after your stay in hospital. We offer you advice and support to make this time after your stay as smooth as possible. 

In close cooperation with the treating medical team, the nursing staff and the physiotherapy staff, the best possible range of care is determined for you and organized if necessary. Our aim is to work with you and your confidants to ensure individual care at home or in a continuing care facility.

• Contact with external service providers such as domestic aid, home nursing, meals on wheels, day centers or 24-hour care 
• Organization of necessary aids such as care beds or walking aids
• Support with various applications such as residential and nursing homes, care allowance, and rehabilitation measures
• Social counseling on financial support options and legal matters for people with care and support needs in complex social situations
