For price information, please contact your non-SHI doctor (“doctor of choice”) or their practice directly. The private doctors/practices know our rates and can tell you the estimated total price including all doctor’s fees. If a cost estimate is required, the non-SHI doctor or practice will provide all necessary information to our billing department.
